The share of medical equipment in government institutions that needs to be replaced was 24.8%, the Ministry of Health told Izvestia. At the same time, in 2021, when the federal project was launched, depreciation was 35.1%. The program was supposed to end in 2024, but it was extended until 2030. The main problem in replacing medical equipment is the lack of a number of domestic—made devices in the country, experts say. How much worn-out medical equipment is there in the public sector

The share of medical equipment used in hospitals and polyclinics with a service life of more than 10 years by the end of 2024 was 24.8%, the press service of the Ministry of Health told Izvestia. The replacement of equipment has been carried out since 2021 as part of the federal project "Modernization of primary healthcare in Russia". At the time of the start of the equipment replacement program, its share in hospitals was 35.1%.

But, as follows from the response of the Ministry of Health, the results are lower than planned. According to the targets reflected in the passport of the federal project, in 2024 it was necessary to purchase more than 48 thousand units of medical equipment, and in 2025 — more than 66 thousand.

"In 2024, as part of the federal project, the regions purchased and commissioned more than 41,000 units of medical equipment, including more than 1,400 "heavy" (large equipment. — Ed.), the ministry clarified. — As of October 1, 2025, the regions have purchased and commissioned more than 40 thousand units, including more than 2 thousand "heavy" ones.

The federal project started in 2021 and was supposed to be completed in 2025. At that time, it was planned to purchase medical equipment in the amount of more than 212 thousand units. In total, about 550 billion rubles were allocated from the federal and regional budgets. At the end of 2024, Minister of Health Mikhail Murashko announced the allocation of about half a billion more rubles to extend the state program until 2030.

"We are implementing a number of systemic measures within the framework of national projects: the first is the continuation of the modernization of the primary sector. We must bring all outpatient clinics in the country to uniform high standards.: This includes a diagnostic base, a medical base and, of course, the condition of buildings and structures," he said at the 19th National Congress of Therapists.

By 2030, the Ministry of Health plans to purchase about 62 thousand more units of medical equipment. But, as Izvestia found out, after examining the passport of the federal project, there are no targets for this item.

Why are medical equipment being slowly replaced?

In Russia, only about 25-30% of medical equipment has been replaced since 2022, said Stanislav Goldberg, chairman of the Opora Russia Medical Device Management Commission.

— Replacement of consumables with domestic ones in the country is faster than medical equipment. Medical equipment in the country is also being replaced, but the process is slower, although all conditions have been created for this. This is due to the fact that, from a technological point of view, it is more difficult to replace it, it takes more time and resources," he stressed. — at the same time, new equipment has been appearing in the country in recent years. For example, X-ray and ultrasound machines. But not at the pace we would like.

"CT, MRI and X—ray machines, as well as laboratory equipment, are currently in the greatest demand in the public sector," they noted.

According to MDPro consulting company, the volume of state purchases of medical equipment in the first half of 2025 remains approximately at the same level as last year.

For example, 96 MRI machines were purchased in 2024, and 41 in the first six months of 2025. Last year, 371 portable X-ray machines were purchased, and 168 in the first half of this year. In 2024, 2.9 thousand were purchased. Ultrasound machines, in January – June 2025 - 1167. At the same time, they noted that there are no Russian MRI machines on the market, and only imported ones were purchased during both periods. But the share of Russian ultrasound machines increased by 18%.

The association "League of Participants in the sphere of medical device circulation" noted that the dynamics of the implementation of the federal project is positive, but if such rates continue, then it will take about five more years to reach, for example, the level of 10%.

In 2025, purchases of medical equipment are inferior to last year's. But at the same time, the overall situation remains stable and meets the needs of medical organizations in it, said Alexey Vanin, CEO of MDPro.

"If we analyze each category separately, we can conclude that this year there will be fewer, for example, C-arc X-ray machines, mammographs, MRI machines and fluorographs," he explained.

According to him, the decrease in purchases of MRI devices is due to the lack of domestically produced devices in the country and difficulties in payment and logistics. Other devices may not be purchased due to the lack of need for them after purchases last year.

At the same time, the share of domestic manufacturers has been increasing by 20% annually over the past few years, said Konstantin Zotov, CEO of Medical Systems and Technologies.

The press service of the Ministry of Health reported that in 2024, of the replaced equipment, the share of Russian-made machinery was more than 57%, in 2025 — more than 63%.

How the equipment is being replaced in the regions

The regions are conducting regional modernization programs at the expense of the federal budget, as well as at their own expense. Izvestia sent requests to the Ministry of Health of all Russian regions with a request to provide information on how much and what equipment was replaced in 2024 and 2025.

The press service of the Ministry of Health of Chuvashia reported that 280 units of equipment for 18 medical organizations were purchased and commissioned as part of the federal project from 2019 to 2024, and 120 more units for 17 medical organizations will be purchased in 2025.

In 2024, almost 4.1 thousand units of medical equipment were installed in 36 medical institutions in the Trans-Baikal Territory.

"The plan for last year was fully implemented," the regional department stressed. "The percentage of worn—out equipment in the Trans-Baikal Territory is about 40%."

By the end of October 2025, almost 1.5 thousand units of equipment had been purchased and installed in 21 medical institutions in this region. The priority is to purchase laboratory medical equipment for diagnostics and for equipping X-ray departments (X-ray machines, mammographs, fluorographs).

In the Ulyanovsk region, the volume of purchases of medical equipment in 2025 increased by 29%, the regional Ministry of Health told Izvestia. In total, 126 units of medical equipment were purchased for 31 medical organizations out of the planned 150. However, they admitted that 55% of medical equipment in the region is completely worn out.

The St. Petersburg Health Committee reported that in 2025, 1,369 units of equipment out of 1,396 were delivered to 79 medical organizations as part of the state program.

"The remaining 27 units are planned to be delivered by the end of the year," they added.

Last year, according to the committee, 74 medical organizations received full equipment, totaling 1,071 units. They added that now the share of equipment with a service life of more than 10 years in hospitals and polyclinics is about 23%.

In Khakassia, seven medical organizations were replaced and retrofitted by the end of October 2025.

"In 2025, heavy equipment was replaced and equipped, and equipment necessary for equipping medical organizations was purchased," the press service of the Ministry of Health of Khakassia clarified.

They noted that every year the regional department collects applications from medical organizations. The next replacement of medical equipment is planned for 2026.

The Moscow Department of Health did not say how much equipment was worn out in the capital's medical institutions, but they said that in 2024 they supplied more than 19.5 thousand. medical equipment units, of which more than 780 are "heavy".

"In particular, there are 24 computer and 30 magnetic resonance imaging units, 13 angiographs, 144 X-ray diagnostic and 42 C-arc X-ray machines, 47 mammographs, 11 densitometers and 426 ultrasound diagnostic systems," the press service of the DZM said.

What's the priority?

To maximize the effectiveness of the program and improve patient safety, priority should be given to intensive care, diagnostic and sterilization equipment, the failure of which directly affects people's lives and health, as well as the system's ability to provide timely and high-quality medical care, according to the League of Participants in the Field of Medical Device Circulation.

— For example, intensive care, sterilization and for the diagnosis of acute conditions. As well as equipment for routine, but massive and critically important diagnostics — laboratory, imaging or functional, they believe.

The replacement of medical equipment should be carried out with an integrated approach, the experts added.

First of all, to change the equipment of the highest priority, which is out of order or whose wear exceeds all permissible standards. For example, 80-90% and above," they stressed.

Priority can also be given to those institutions where replacing one key device will cover the needs of an entire district and reduce the need for patients to travel to the regional center. For example, in rural areas, the association explained.

In addition, it is necessary to replace equipment where there are already trained personnel for it — specialist doctors, laboratory technicians and renovated premises.

Equipment purchases by private companies

The main contribution to the demand for medical equipment is created by government agencies, Konstantin Zotov believes. But private clinics, he said, have also begun to pay more attention to the quality of medical equipment. Alexey Vanin agreed with him.

"The acquisition of medical equipment by private companies is influenced by new trends in healthcare: the demand for ophthalmological and dental services that involve a personalized approach," he added.

Alexey Kanevsky, Deputy General Director for Operations at Euroonco Group, confirmed to Izvestia that private organizations are constantly purchasing and implementing new equipment, as attracting patients requires high, expert quality of care, especially in the high-tech sector.

Demand for medical equipment has grown by 6% over the year, analysts of the Avito platform confirmed to Izvestia.

"There is a trend in pharmaceutical, ophthalmic, surgical, and dental medical equipment in the subcategories," they clarified.

The increase in medical equipment shipments in the third quarter of 2025 was confirmed to Izvestia by the press service of the Business Lines Group of Companies.

— Relative to the second quarter, it grew by 13%, — the company specified.

They noted that Central Russia is the leader in shipments and receipts among the regions. It accounts for 62% of shipments from the total medical equipment turnover and 25% of receipts. The Volga region ranks second: 9% of shipments and 14% of receipts.
